Insurance lapse & cancellation notices

When an insurer terminates a carrier's FMCSA-filed coverage, it must notify FMCSA ahead of the effective date, so the public record shows cancellations before they land. If the effective date arrives with no replacement filing, the carrier's required coverage has lapsed, and FMCSA revokes operating authority for failure to maintain insurance. This chain is the most common way carriers lose authority.

Reading a pending cancellation

A pending cancellation is a dated warning, not a verdict. Many are resolved before the effective date: the carrier switches insurers and a replacement filing appears, or the notice is withdrawn. The closer the effective date, the more urgent the question "is replacement coverage in place?" becomes. fleetfax surfaces pending cancellations with the effective date and treats one already in effect with no replacement as a blocking issue.

Two distinctions that matter

First, a policy reaching its normal term end and renewing is not a cancellation, even though both produce dated entries in the record; reading every dated entry as a lapse manufactures false alarms, so fleetfax keys on processed cancellation events, not on policy anniversaries. Second, one lapse years ago is a different fact from a carrier that cycles insurers constantly; repeated cancellations in a short window form their own pattern, and the report counts them separately from today's coverage status.
